Empowering APPs and Other Clinicians to Improve Migraine Management

Migraine is one of the most common debilitating medical conditions, yet there is a shortage of healthcare providers trained in the management of the condition. Clinicians across medical specialties play a crucial role in addressing this unmet healthcare need.

This program is designed to help advanced practice providers and physicians gain the clinical experience and confidence to diagnose and treat migraine. There are two types of apprenticeship sites available: headache clinics and ENT/vestibular centers. While there will be some overlap between the two, headache clinics primarily focus on the diagnosis and treatment of migraine, including rare and refractory presentations. This apprenticeship is particularly suitable for clinicians in primary care, obstetrics and gynecology, neurology, pain management, or related fields.

In contrast, ENT/vestibular centers will emphasize the otolaryngologic manifestations of migraine and the evaluation and management of vestibular conditions, including vestibular migraine. This apprenticeship is well-suited for clinicians in balance centers, otolaryngology, surgical otolaryngology, audiology, neurotology, or other related fields.

The Migraine Apprenticeship program offers healthcare providers and advanced practice providers from any specialty the chance to gain hands-on experience by shadowing migraine specialists at locations throughout the United States.

Program Information

This program is limited to U.S.-based health care providers only.

Previous experience in diagnosing and treating migraine is not required.

Clinical sites’ subspecialties include headache, neurotology, pediatric otolaryngology, and general otolaryngology.

Learning Objectives (these vary based on site)

Understand key mechanisms in migraine pathophysiology.
Identify ENT manifestations of migraine (ENT sites only).
Apply evidence-based pharmacologic and non-pharmacologic treatment strategies for acute and preventive migraine management.
Understand the mechanisms, indications, and side effects of common migraine medications, including abortive and preventive therapies.
Assess when to escalate or modify treatment based on efficacy and side effects.
Recognize the impact of diet, exercise, sleep, and stress management on migraine management.
Learn indications and techniques for botulinum toxin, SPG blocks (ENT sites only), occipital nerve blocks, and other nerve blocks.
Master the documentation of migraine diagnosis, treatments, and outcomes for clear patient records and insurance claims.
Learn strategies for navigating insurance authorizations for advanced therapies and imaging when necessary.

Program Details

This is a 4.5-day program. On Tuesday mornings, Dr. Joshi will not be available in the outpatient clinic. The rotation will start at 8:30 AM and end at 4:00 PM. There is a break from noon to 1 PM, where Dr. Joshi and the apprentice can incorporate some didactics.

On all of the days, activities will include:

Shadowing during new patient visits, follow-ups, and procedure visits
Learning key diagnostics and pharmacological treatment options
Integrating non-pharmacological options
Didactic sessions that pertain to individual cases
Potentially presenting with colleagues to see other Neurological cases

The clinic is conveniently located off Route 9 in Westborough, Massachusetts, approximately 40 minutes from Worcester and about one hour from Boston. Logan Airport is roughly a one-hour drive from the clinic.
There are multiple hotels within a 15-minute drive of the clinic, along with a wide variety of nearby dining and entertainment options throughout the Westborough and Framingham areas.
There is free parking at the clinic.

This is a two-day program. Apprentice will see patients together with Dr. Kuruvilla and discuss diagnostic and therapeutic approaches to headache management.

An apprentice would participate in joint patient visits focused on evaluation and treatment planning for headache disorders. We can collaborate on research if there is interest.

Shadowing a headache specialist APP, with opportunities to shadow other providers (MD/DO) for related or other neurological conditions (the clinic has an epileptologist, neuro-ophthalmologist, pediatric neurologist, MS specialist and memory/vascular specialist)
The option to observe procedures (Botox, nerve block/TPI, infusions, in-office acute injections)

Most hotels in Eugene/Springfield are within a very drivable distance of the office.
There are many great food truck options and locally sourced, farm-to-table options.
Eugene Airport is local to the area. There is a 1 hr 45 min to the Portland airport, and a good bus/public transport system. Can get anywhere in Eugene/Springfield within 30 min or so.

This clinical rotation with Dr. Godley will primarily focus on the diagnosis and management of migraine variants. It will also include observing procedures such as onabotulinumtoxinA injections, sphenopalatine ganglion blocks, and occipital nerve blocks.

Day 1: The morning will be unscheduled, allowing for potential travel time or a didactic session based on the applicant’s preference. In the afternoon, from 1:00 to 5:00 PM ET, the applicant will attend a clinic at the University Otolaryngology in East Greenwich, RI.
Day 2: Option to have a didactic morning session from 8:30 a.m. to noon, followed by an afternoon clinic from 1:00 to 5:00 PM, or a full day in the clinic from 8:00 AM to 5:00 PM ET.
Day 3: The apprentice will spend the day in the clinic from 8:00 AM to 5:00 PM ET, or can combine with the Boston Children's Hospital Balance and Vestibular program.

The apprentice can combine the University Otolaryngology clinical rotation with an additional rotation at Boston Children's Hospital Balance and Vestibular Program.

The LOOM Hotel is located in Warwick, Rhode Island, and is a 10-minute drive from the East Greenwich clinic and a 15-minute drive from the Providence location. T.F. Green International Airport is a 10-minute drive from the hotel.
The surrounding areas around the clinic sites have great options for food and entertainment, including Main Street in East Greenwich, Federal Hill in Providence, the Providence Performing Arts Center, WaterFire, and the RISD museum.

Program Details

The Balance and Vestibular Program at Boston Children’s Hospital (BCH) is located in Waltham, Massachusetts, approximately 15 miles from the main campus in Longwood. Vestibular migraine is the primary migraine type diagnosed and managed within our program. At this time, limited procedural interventions in this program. Candidates interested in both procedural interventions for migraine management and pediatric vestibular migraine management should consider applying for the joint apprenticeship between BCH and University Otolaryngology in RI.

Day 1: The apprentice will spend the day in the clinic with a pediatric otologist and a pediatric nurse practitioner, diagnosing and managing patients with various pediatric vestibular disorders, including, but not limited to, vestibular migraine, pediatric migraine variants, balance impairment, and motion sickness. There will be an opportunity to follow a patient through the multidisciplinary clinic, which includes vestibular testing, a medical appointment, and an evaluation by physical therapy.
Day 2: The apprentice will spend the morning in the Multidisciplinary Concussion Clinic. They will follow a patient as they meet with clinicians from various specialties, including psychology, otolaryngology, sports medicine, physical therapy, ophthalmology and optometry. They will spend the afternoon with the psychologist in the program. The psychologist specializes in the treatment of vestibular disorders, including migraine and pain disorders, and uses Cognitive Behavioral Therapy and Biofeedback for the treatment of these disorders.
Day 3: The apprentice will spend the day in clinic in collaboration with a vestibular nurse practitioner diagnosing and managing vestibular disorders, primarily vestibular migraine.

Additional Opportunities

During the course of the fellowship, the participants will have opportunities to hear presentations and participate in discussion on pediatric vestibular migraine management.
The apprentice will have the opportunity to spend time observing various members of the multidisciplinary team who collaborate in managing pediatric patients with vestibular migraine, including pediatric physical therapists, a psychologist, and an audiologist who performs vestibular testing.

Following this, the apprentice will visit the Balance and Vestibular Program at Boston Children’s Hospital (BCH), located in Waltham, Massachusetts, approximately 15 miles from the main campus in Longwood. Within this program, vestibular migraine is the primary type of migraine diagnosed and managed. This clinical site offers limited procedural interventions.

This two-day clinical rotation with Dr. Teixido will focus on diagnosing and managing migraine variants, hearing conditions, and balance disorders. The apprentice will observe procedures such as onabotulinumtoxinA injections, sphenopalatine ganglion blocks, and occipital nerve blocks.

Day 1: In the afternoon, from 1:00 to 3:30 PM, the apprentice will attend a clinic with Dr. Teixido, followed by a didactic session on 'Migraine in ENT' with a Fellow from 3:30 to 5:30 PM.
Day 2: The day begins with a clinic from 8:00 AM to 12:00 PM, followed by a lunch break from 12:00 to 1:00 PM. In the afternoon, a procedures clinic focused on onabotulinumtoxinA and nerve blocks will be held from 1:00 to 5:00 PM.

This three-day clinical rotation at the University of Colorado will offer the opportunity to shadow at multiple clinic sites, including neurology and ENT, with a focus on neurotology, rhinology, and vestibular disorders. The rotation will also include a day of observing procedures such as onabotulinumtoxinA injections, sphenopalatine ganglion blocks, and occipital nerve blocks.

Day 1: Shadow the neurology/ENT clinic, which focuses on vestibular-related conditions.
Day 2: Shadow the ENT clinic, which focuses on neurotology and rhinology.
Day 3: Observe procedures in the neurology clinic.

This two-day clinical rotation at the University of San Francisco will offer the opportunity to shadow neurotologist, Jeffrey Sharon, as well as a vestibular APP, Kali Koziol. The rotation will also include a day of observing procedures such as onabotulinumtoxinA injections, sphenopalatine ganglion blocks, and occipital nerve blocks.

Day 1: Shadow the neurotology with Dr. Sharon in the East Bay.
Day 2: The apprentice will shadow Kali, an APP who specializes in vestibular disorders, with tele-visit appointments in the morning and in-person clinic appointments in the afternoon in San Francisco.

This two to four-day clinical rotation with Dr. Djalilian and his APP will focus on diagnosing and managing migraine variants, hearing conditions, and balance disorders.

Day 1 (Monday): All-day clinic session with Dr. Djalilian and his APP.
Day 2 (Tuesday): All-day clinic session with Dr. Djalilian’s APP, who primarily sees otologic migraine patients (tinnitus, dizziness, hearing loss, aural fullness, etc.).
Day 1/3 (Optional or alternative to Mon/Tue Schedule) Thursday: All-day clinic session with Dr. Djalilian and his APP.
Day 2/4 (Optional or alternative to Mon/Tue Schedule) (Friday): All-day clinic session with Dr. Djalilian’s APP, who primarily sees otologic migraine patients (tinnitus, dizziness, hearing loss, aural fullness, etc.).

2025-26 Available Dates

All weeks except for Thanksgiving week and holidays, as well as the week of October 9-13.

Program Details

Our multidisciplinary clinic offers expertise in managing various vestibular disorders, including vestibular migraine and Meniere's disease. The clinic is located in the same space as a vestibular lab, clinic, and physical therapy facility. We have two APPs (one NP and one PA) who are experienced in managing vestibular disorders and vestibular migraine (trained by us).

An apprentice would learn to comprehensively evaluate a patient presenting with dizziness, interpret vestibular exam and vestibular lab findings and build a treatment plan including medication and physical therapy and/or procedures as indicated. We do Botox and external nerve blocks. We rarely do SPG blocks. We occasionally do Botox for TMJ, but we don't offer a comprehensive TMJ management (there is a dedicated orofacial pain clinic in the dental school down the hall from me, so I don't manage those). We do a lot of intratympanic steroid injections for Meniere's disease, which can overlap with migraine disorders.
